Expectations, Policies & Procedures
Below are the expectations, policies and procedures provided to all clients related to personal training, both in small group classes and private sessions. If there are any questions, concerns and/or feedback, please reach out using the contact information at the bottom of this page.
Liability Waiver
Acknowledgment of Risk
I hereby understand and acknowledge that the training, programs and events held by Self Made Training Facility may expose me to many inherent risks, including accidents, injury, illness, or even death. I/We assume all risk of injuries associated with participation including, but not limited to, falls, contact with other participants, the effects of the weather, including high heat and/or humidity, and all other such risks being known and appreciated by me.
Voluntary Participation
I voluntarily agree to participate in the personal training sessions provided by the Trainer. I understand the nature of the activities I will be participating in, and I have been given the opportunity to ask questions regarding any aspect of these activities. I agree to follow all instructions and guidelines provided by the Trainer to minimize the risk of injury.
Medical Clearance
I certify that I am in good health and physically able to participate in personal training sessions. I have disclosed any medical conditions, injuries, or limitations that may affect my ability to participate. I understand that the Trainer is not a medical professional and that it is my responsibility to consult with a physician prior to beginning any exercise program.
Release of Liability
In consideration of being allowed to participate in personal training sessions, I hereby release, discharge, and hold harmless the Trainer, their agents, employees, representatives, and affiliates from any and all claims, demands, damages, rights of action, or causes of action, present or future, arising out of or connected with my participation in personal training sessions, including any injuries resulting therefrom.
Indemnification
I agree to indemnify and hold harmless the Trainer and/or ownership of any facilities being used from any and all claims, actions, suits, procedures, costs, expenses, damages, and liabilities, including attorney's fees, arising out of or related to my participation in personal training sessions.
Assumption of Risk
I understand that there are potential risks associated with physical activity and exercise, including but not limited to muscle strains, sprains, and other physical injuries. I assume full responsibility for any and all risks associated with my participation in personal training sessions. I hereby acknowledge my responsibility in communicating any physical and psychological concerns that might conflict with participation in activity.
Emergency Medical Treatment
In the event of an emergency, I authorize the Trainer to obtain medical treatment for me, if deemed necessary. I understand that I am responsible for all costs associated with any medical treatment that I may receive.
Severability
If any provision of this Waiver is held to be invalid or unenforceable, the remaining provisions shall continue in full force and effect.
Governing Law
This Waiver shall be governed by and construed in accordance with the laws of California.
By signing up for a class, consultation and/or private session, I acknowledge that I have read and understood this Waiver, and I agree to be bound by its terms. I understand that by signing this document, I am waiving certain legal rights, including the right to sue the Trainer.
Policies & Procedures
Expiration Policy
Purchased training sessions/classes expire anywhere from 30 days to 3 months after the date of purchase. Any sessions that remain after the expiration date will be forfeited. If you are physically unable to continue training, you must bring a doctor’s note. Any prolonged travel plans you must make special arrangements in advance.

Purchased memberships under Personal Training and/or purchased individual private sessions are valid for 3 months.

Purchased class packs are valid for 3 months.

Purchased memberships under Group Class are valid for 30 days.
Tardiness Policy
You are expected to begin working out at the start time of the scheduled appointment. A late start time does not entitle you to a session longer than the scheduled appointment. For example, if you are 20 minutes late for a 60 - minute training session, your session will be reduced to 40 minutes and you will not receive credit for the remaining 20 minutes.
Cancellation and Rescheduling Policy and Procedure
As a dedicated trainer, I’m committed to helping you reach your goals with consistency and focus. In order to provide the best service and maintain a structured schedule, I ask that you respect the following policy:

Notice Requirement: A minimum of 12 hours’ notice is required for cancellations or rescheduling. Cancellations made within 12 hours of your scheduled class will result in a full charge for the class.

Late Cancellations & No-Shows: If you miss a class without prior notice (no-show), or cancel within 12 hours of your scheduled time, the class will be charged in full.

Monthly Billing: Monthly billing clients are required to give 15 days’ notice prior to the end of the month to cancel or pause the next month’s billing. If notice is not provided, the upcoming month’s payment will be processed and/or due as scheduled.

Emergency Exceptions: In case of unforeseen emergencies or medical reasons, please reach out as soon as possible. I will assess each situation on a case-by-case basis.
